The following packet includes the Physical, Concussion and Participation forms (below). We also require a copy of your insurance card for proof of insurance. These are for students participating in all athletic activities. If you will have more than one child participating you will have to fill out the form for each child. This form must be completed, signed, and submitted online before your student may practice or participate in an athletic event. If you have any questions, please contact the athletic office at 712-792-8010, ext 1001.
